I allow my child to participate in all activities at the YOUTH SPORTS PROGRAMS. I am fully aware that injuries may occur in any activity. Therefore, in the event my child is injured within the before or after school, camp, before and or after camp programs, I hereby release YOUTH SPORTS PROGRAM, the City of Baltimore, Baltimore City Schools, Baltimore City Recreation and Parks their agents and employees from any and all claims or liability for any damages or injuries which may be sustained. I will assume full responsibility for any injuries incurred.
